CFSA now requires a school enrollment packet for children attending schools in any jurisdiction, including Washington, DC. All packets require approval by a CFSA Education Specialist in Clinical Practice. Here is what you need to know to ensure a smooth process:
Who Needs An Enrollment Packet?
- All out-of-home school aged children attending Public, Charter, or Private Schools
When Should a Social Worker complete an enrollment packet?
- Always at the beginning of every school year
- Always when child changes schools
How to Complete a School Enrollment Packet?
- Update FACES education screens reflecting the school the child will be attending.
- Complete the school enrollment packet:

- Student Information Form
- Copy of the most recent court order
- County/Jurisdiction form requirements. (see checklist below)
DC Student Information Change Form [DOC] - Must be completed when any of the following occurs: guardianship is granted for a child/youth, an adoption petition is filed, an adoption petition is withdrawn, a change in a child/youth’s educational decision-maker, and/or a foster care/group home placement change. The form must be submitted to the CFSA Education Specialist within 48 hours of the change.
