Visitation Planning

· Quick Reference: Instructions on how to use the Visitation Templates

·  In-Home Visitation Template is to be used for families that are receiving in-home services

· Out-of-Home Visitation Template is to be used for children that have been temporarily removed from the care of their parent(s) or caretakers.

Both of these templates should be used to guide the visitation planning with the children/families on your caseload. The purpose is to ensure each visit is meaningful and constructive and all parties involved are getting the most from this time. The tool is to be used every 90 days or sooner (if there are changes needed) and in conjunction with the family. Please refer to the Quick Reference Guide for further information.
