Social Workers:
You must complete a tuition enrollement packet for all children in care who attend school outside Washington, DC. At a minimum, the tuition enrollment packet includes:
- County Tuition Contract
- Special Education and School Tuition Authorization Report
- Individualized Education Plan (IEP) *(Only if the child receives special education services)
Below is a list of required documents to complete your enrollment package. To get started, click on the county/city name to open the tuition contract for that jurisdiction. Print the document, fill it out and bring it to CFSA, 400 6th Street, SW, Room number 4178 between 9 am - 3:30 pm, from August 10 through August 14. Did you know?
The
D.C. Student Information Change Form (Word doc) should be completed when a child experiences any change to their case status including, but not limited to, the social worker, the social worker’s supervisor, foster care placement, legal status, educational decision-maker, adoption petition status, etc. The form should be completed and submitted immediately, but no later than ten (10) days of the change.
