General CPR Check The Child and Family Services Agency (CFSA) provides Child Protection Register checks for employers that provide services to children/youth. Employers may request CPR checks for prospective and current employees as part of the hiring/continued employment decision making processes. Click on this link for the General CPR Check Application form*. Requests shall be processed within 30 days from receipt of the application.
CPR Check for Foster and Adoptive Home Licensing CFSA provides CPR checks for child placing agencies requiring checks as part of a foster/adoptive home licensing process. (This application is to be used by DC licensed child placing agencies for requesting expedited CPR checks for temporary licensing of kin only. These requests shall be processed within 3 hours of receipt of the application.)
History Check for Child Protective Service Agencies from Other Jurisdictions CFSA responds to requests for historical information submitted by governmental Child Protective Service Agencies from other jurisdictions in relation to child protective service investigations being conducted in another jurisdiction. Representatives from governmental Child Protection Service agencies in other jurisdictions can fax a written request on agency letterhead to the Supervisor of the District’s Child Protection Register (CPR) Unit at (202) 727-8040. Requests should include the full name, date of birth, social security number and any DC addresses for the person(s) being checked along with the name, title and telephone number of the requestor. CFSA will respond within 24 hours as to whether the subject(s) have a history with the District’s child welfare agency. Detailed information in relation to any history with the agency back to 1999 shall be provided in writing to the requesting jurisdiction within 3 to 5 business days following receipt of the request.
