The 2021 Annual Child Fatality Review Report, originally published on January 30, 2023, was corrected to take out the first sentence in the second paragraph on page 40, which was not related to the section. The following sentence was removed: “While 41 percent (n=12) of the families had direct CFSA involvement at the time of the fatality, 59 percent (n=17) were not involved with CFSA at the time of the fatality.” Additionally, table 8 on page 41 originally included the column headings (reports). The headings are now removed. Lastly, “Screen-Outs” have been revised for clarification to “Screened-Out Reports.”
