This policy establishes responsibilities for managing privacy risk in creating, collecting, maintaining, using, storing, transmitting, protecting, and destroying Personal Identifiable Information (i.e., name, Social Security Number, biometric records, date and place of birth, mother’s maiden name). This policy ensures that users understand their responsibilities to protect data, including electronic protected health information (“ePHI”) that is stored in or transmitted through FACES or other Agency management information systems.
